HX of alcohol abuse, CHF, Coumadin therapy
Patient went through windshield in car accident and landed on asphalt - Broken bones, contusions, requiring extended hospitalization - De-gloved left hand with tendon involvement
Wound cleansed prior to DermaFill application
- Patient refused to have ring removed
Day 4
- Dressing in place and ring became loose, in fact wiggling in place
- Patient continued to state hand was free of pain
Day 7
- Dressing removed by staff fearing underlying exudate was infection
- Wound revealed lack of signs and symptoms of infection and presented with a clean granular wound bed
Day 9
- Dressing replaced x 48 hours
- Note: Plasma and other bioactive agents accumulate under dressing
Day 12
- The dressing is re-sealed and clear, allowing an unobstructed view of the healing process
- Note: return of veins
4 week follow-up
- DermaFill still clear allowing staff to chart progress
- As the wound heals and contracts, the dressing liberates itself
11 week follow-up
- Patient states last of dressing fell off three weeks earlier
